All three of these athletic forms must be turned in at the beginning of the year for your child to be eligible to participate in our Christian Academy Athletics Program.  If your child has any interest or is planning to participate in any of the great sports offered at CA,  please print each form, fill them out completely, have a sports physical done (preferably over the summer) and turn them all in on the first day of school.  These forms must be turned in each year and a new physical form must be filled out by your child's physician each year as well.   If you are unable to print these forms there are copies in the Main Office.
